Breast cancer hormone replacement therapy Epidemiological observations have shown clear relationships between a woman's reproductive history and the risk of cancers of the sex organs, which are generally thought to reflect changes in hormonal secretions; but which hormones are concerned and the mechanisms by which they act are, for the most part, still uncertain. An exception is endometrial cancer, the risk of which is directly related to the degree of exposure to oestrogen not followed after an appropriate interval by progestogen. Proof that oestrogenic stimulation of the mammary tissues is a cause of most cases of breast cancer in developed countries has been provided by randomized trials of tamoxifen, an antioestrogenic drug that blocks the oestrogen receptors in the cells of the normal breast. The effect is large and rapid: 5 years of tamoxifen approximately halves the incidence of breast cancer not only while the drug is being taken but also for some years afterwards.
